## Summary For Families

The goal is to collect real-world experiences from people affected by Parkinson's so the Parkinson's Foundation can identify unmet needs, shape future research priorities, and improve programs and services. The approach is non-interventional surveys, asking people with Parkinson's, their care partners, and clinicians about symptoms, treatments, daily challenges, and research interests, and the aggregated answers are used to guide funding decisions and the design of future studies. Anyone 18 or older who self-reports a Parkinson's diagnosis, is a care partner for someone with Parkinson's, or is a health professional who regularly works with people with Parkinson's, and who is willing to be contacted, is eligible to participate.

- **Minimum age:** 18 Years
- **Sex:** ALL

Inclusion Criteria:

* Willingness to be contacted by the Parkinson's Foundation for surveys and any future research opportunities
* Anyone who self-reports that they have a diagnosis of Parkinson's disease
* Care partner of a person with Parkinson's disease
* Any health professional that regularly works with people with Parkinson's disease
* 18 years of age or older

Exclusion Criteria:

* People who report that they do not have a diagnosis of Parkinson's disease, are not a care partner to someone with PD or do not work with people with PD in the health setting
* An individual younger than 18 years of age
