Helping people with Parkinson's start walking more smoothly, this trial targets the small balance and weight-shift problems that make the first step hard and can lead to freezing or falls. It combines explicit weight-shift and anticipatory postural adjustment training with real-time feedback, plus paired associative stimulation, a noninvasive technique that pairs brief nerve pulses with brain stimulation to strengthen the motor pathways that control stepping. The interventions are rehabilitation and neuromodulation, not drugs, so they do not directly change levodopa dosing. Adults aged 20 and up with a clinical diagnosis of Parkinson disease are eligible, while people with leg musculoskeletal injuries, osteoporosis, or other peripheral or central nervous system diseases are excluded; about 100 participants will be enrolled.
