## Summary For Families

The goal is to test whether a combined motor and cognitive exercise program can be delivered safely and help with common Parkinson's problems like gait, balance, attention, and multitasking. The program pairs physical movements, such as walking or balance drills, with cognitive tasks like memory or attention challenges to train the brain networks that control both mobility and thinking; it is a non-drug approach meant to complement medications like levodopa and is compared to usual care. The trial is enrolling adults 18 and older with Parkinson's by MDS criteria, with symptoms for at least four years, who can walk independently, and it excludes those with Parkinson's disease dementia.

Inclusion Criteria:

* Diagnosis of PD according to MDS-PD criteria
* Aged ≥ 18 years
* Informed consent
* PD symptoms ≥ 4 years
* Independent gait

Exclusion Criteria:

* Diagnosis of PD Dementia according to the MDS-PD Dementia criteria
